Your insurance situation changes significantly, especially when you transition from unemployment to self-employment with a startup grant.

In my work as a startup coach, this is therefore a recurring topic, especially for entrepreneurs who are starting a business with a startup grant while receiving ALG I benefits. In this article, you’ll learn about the options available to you as a self-employed person, how statutory health insurance premiums will be calculated in 2026, and what you should keep in mind when transitioning from receiving ALG I benefits.

Note: This article is for general informational purposes and has been carefully researched. It is not a substitute for individual insurance, legal, or tax advice. Contributions, assessment thresholds, and insurance terms and conditions are subject to change. As of August 2026.

Health Insurance Before Starting a Business While Unemployed

As long as you are receiving Unemployment Benefit I, you generally remain covered by health and long-term care insurance. The Federal Employment Agency covers the corresponding premiums while you are receiving benefits.

This situation changes when you begin working as a full-time self-employed person and your unemployment benefits (ALG) end. From that point on, you must pay for your own health and long-term care insurance, and you should determine well in advance how your insurance coverage will continue.

What happens to health insurance after the startup grant is approved?

If you are using the start-up grant and your entitlement to unemployment benefits ends, your health insurance coverage through the Federal Employment Agency generally ends as well.

If you were previously covered by statutory health insurance, your membership generally continues as voluntary membership in the statutory health insurance system. Withdrawal is possible only under certain conditions and, in particular, requires that you have other health insurance coverage.

Therefore, notify your health insurance provider well in advance that you are starting your own business and clarify what information and documentation they require for your classification. Especially when starting a business, many organizational tasks are happening at the same time, so this detail can easily be overlooked.

What health insurance options are available to you as a self-employed person?

If you become self-employed as your primary occupation and are no longer required to be enrolled in the statutory health insurance program, you generally have two options: You can remain in the statutory health insurance program (GKV) on a voluntary basis or switch to private health insurance (PKV).

Voluntary statutory health insurance offers the advantage that premiums are generally based on your income subject to contributions. There is no medical examination. In the next section, we’ll take a closer look at how much the premiums for self-employed individuals will be in 2026.

With private health insurance, however, your premium is not based on your income. Factors such as your age, your health status, and the plan you choose are key. That’s why you shouldn’t base your decision to switch solely on your current monthly premium.

Non-contributory family health insurance is generally not an option for those who are self-employed as their primary occupation. However, it may still be possible under certain conditions for those who are self-employed as a secondary occupation. The health insurance provider determines on a case-by-case basis whether an activity is considered a primary or secondary occupation.

Special rules also apply to certain occupational groups. For example, self-employed artists and journalists may be covered by statutory health and long-term care insurance through the Artists’ Social Insurance Fund.

Statutory Health Insurance Premiums for the Self-Employed in 2026: How They Are Calculated

As a self-employed person with voluntary statutory health insurance, you generally pay your health and long-term care insurance premiums yourself. Unlike employees, there is no employer to cover half of the premium. Health insurance premiums for the self-employed can account for a significant portion of operating costs, especially during the startup phase.

In my work advising startup founders, I consistently find that this cost category is initially underestimated—especially when founders base their estimates on their current ALG I contribution or their previous employee contribution.

What types of income are taken into account?

When you are self-employed , your income from work—that is, the profit calculated for tax purposes— is generally used to determine your contribution , not your revenue.

For individuals with voluntary statutory health insurance, other types of income subject to contributions may also be taken into account. These include, for example, income from capital assets as well as income from renting and leasing.

Premium Rate and Additional Premium

If you are not entitled to statutory sick pay , the reduced contribution rate of 14.0% applies to voluntarily insured self-employed individuals . If you choose to be eligible for statutory sick pay starting in the seventh week of incapacity for work, the general contribution rate of 14.6% applies .

In addition, each health insurance provider charges its own supplemental premium. The average supplemental premium rate set for 2026 is 2.9%. However, the actual premium charged by your health insurance provider may be higher or lower than that.

In addition, there are contributions to long-term care insurance.

Minimum and Maximum Limits for 2026

There is a minimum and a maximum limit for calculating contributions.

The minimum contribution base for 2026 is €1,318.33 per month. Even if your income subject to contributions is lower than this amount, your contribution will generally be calculated based on this amount at a minimum.

The upper limit for calculating contributions is €5,812.50 per month or €69,750 per year. Income above this limit will not further increase your health and long-term care insurance premiums.

The minimum assessment base is particularly important during the startup phase: A low profit does not automatically mean that your health insurance premium will decrease proportionally.

What will be the maximum GKV premium for self-employed individuals in 2026?

When applying the average supplementary contribution rate of 2.9% set for 2026, the maximum base contribution to statutory health insurance is €982.31 per month, excluding entitlement to sick pay.

With a statutory entitlement to sick pay starting in the seventh week and the applicable general contribution rate, the maximum monthly contribution is €1,017.19.

In addition, there is long-term care insurance. The premium for this depends, among other things, on whether you have children and how many. Childless insured individuals aged 23 and older pay an additional premium surcharge.

Therefore, it is not possible to specify a flat “maximum statutory health insurance premium”—including long-term care insurance—as a single amount for all self-employed individuals.

Public Health Insurance (GKV) or Private Health Insurance (PKV): What Should Self-Employed Individuals Keep in Mind?

There’s no one-size-fits-all answer to whether statutory or private health insurance is a better fit for your self-employment. Both systems work differently and can have very different long-term effects on your costs and your coverage.

Therefore, it’s not just a matter of which premium seems more affordable at the time you start your business. Your income, family situation, age, health, and long-term plans also play a role.

That’s why, in my coaching sessions, my goal isn’t to recommend a specific type of insurance. Rather, it’s important that the decision aligns with your projected income growth, personal expenses, and the overall financial plan for your startup.

What are the advantages of voluntary statutory health insurance?

Under the statutory health insurance system, your premiums are generally based on your taxable income, within the applicable minimum and maximum limits.

This can be particularly advantageous when income fluctuates during the startup phase. If your income subject to contributions decreases, your contribution may generally be reduced as well—though not below the amount calculated based on the minimum assessment basis.

A health examination is also not required for voluntary enrollment in the GKV. Under certain conditions, spouses or children may also be covered under family insurance at no cost.

What should you consider when choosing private health insurance?

With private health insurance, your premium is not based on your income. The main factors that determine the premium are the level of coverage you choose and individual factors such as your age at the time of enrollment and your health status.

Private health insurance may therefore initially seem financially attractive to young, healthy self-employed individuals, for example. However, that alone should not be the deciding factor. Benefits, deductibles, premium trends, and personal life plans should also be taken into account when making this decision.

Switching to private health insurance is a long-term decision

In principle, self-employed individuals can switch from voluntary statutory health insurance (GKV) to private health insurance (PKV). However, returning to statutory health insurance at a later date is not an option one can choose freely; rather, it requires that the obligation to be insured under the GKV arise again and that the statutory requirements be met.

That’s why you shouldn’t base your decision to get private health insurance solely on the current monthly premium.

When planning to start your business, it’s especially important to realistically factor in your estimated health and long-term care insurance costs. If necessary, you should consult with your health insurance provider or a qualified insurance advisor to determine which insurance plan or specific coverage option is right for you.

Coverage in the Event of a Prolonged Illness

For the self-employed, a prolonged period of inability to work can quickly become a financial problem. Unlike employees, they generally do not receive continued pay from an employer. That’s why you should also consider how to protect yourself against a prolonged loss of income due to illness when planning your finances.

Sick Pay Under Statutory Health Insurance

Self-employed individuals who are voluntarily insured under the statutory health insurance system generally have no entitlement to statutory sick pay at the reduced contribution rate of 14.0%.

However, you can choose the standard contribution rate of 14.6%. In that case, you are generally entitled to statutory sick pay starting in the seventh week of your inability to work.

In addition, statutory health insurance plans offer special optional sick pay plans. The start date, amount, and cost of this coverage may vary depending on the health insurance provider.

Which option makes sense depends, among other things, on how long you can cover a loss of income using your own savings.

Private Daily Sickness Benefits Insurance and Supplemental Insurance

Another way to protect against loss of income is private daily sickness benefit insurance. The amount of the insured daily sickness benefit, the start date of coverage, the medical exam, and the premium depend on the policy you choose and your personal situation.

In addition, individuals with voluntary statutory health insurance can purchase private supplemental insurance, for example, for dental prosthetics, hospital services, or travel abroad.

Such additional benefits are usually less critical to the immediate planning of starting a business than the question of how your ongoing personal expenses will be covered in the event of a prolonged inability to work.

Here’s How to Keep Track of Your Health Insurance Costs

Statutory health insurance premiums are a significant ongoing expense for self-employed individuals. You should therefore review a few key points on a regular basis—especially during the first few years of your self-employment.

Compare Health Insurance Supplementary Premiums

In addition to the standard or reduced premium rate, each statutory health insurance provider charges its own supplemental premium. This can vary from one provider to another and may change over time.

It can therefore be worthwhile to compare plans regularly. When doing so, however, you should consider not only the premium rate but also the health insurance provider’s coverage, accessibility, plan options, and customer service.

If your health insurance provider increases its supplemental premium, you generally have the right to cancel your policy. To switch to another provider within the statutory health insurance system, you usually don’t have to cancel your current policy yourself: You simply enroll with the new provider, which will coordinate the switch with your current provider.

Keep Track of Tax Assessments and Contribution Determinations

When you start your self-employed business, your health insurance provider does not yet know your actual annual income. Therefore, your premiums are initially set on a provisional basis based on your estimated income.

Once an income tax assessment is issued, the health insurance provider can calculate the premiums on a final basis based on the actual taxable income earned. This may result in an additional payment or a refund.

Therefore, submit the requested tax assessment notices and proof of income to your health insurance provider on time, and take any potential additional payments into account in your cash flow planning.

That’s why I specifically factor in potential additional payments when doing liquidity planning with founders, so that they don’t unexpectedly lead to a cash flow crunch.

Take Action When Your Income Drops Significantly

If your income drops significantly, you shouldn’t automatically assume that your health insurance premium will be adjusted right away.

Under certain circumstances, a lower current income level can be taken into account even before you receive your next income tax assessment. Therefore, be sure to contact your health insurance provider early on if your financial situation changes significantly.

However, the minimum assessment base still applies. Therefore, even if profits are very low, the contribution cannot be reduced arbitrarily.

Make an Informed Choice About Sick Pay Coverage

Don’t just check the amount of your monthly health insurance premium; also check what coverage you have in the event of a prolonged inability to work.

The lower, reduced GKV premium rate generally means that you are not entitled to statutory sick pay. Depending on your financial situation, it may make sense to opt for statutory sick pay, an optional plan, or private daily sickness benefit insurance.

The key factor is how long you could cover your ongoing personal and business expenses using your own savings.

Plan Your Switch to a New Health Insurance Provider Well in Advance

As a general rule, a 12-month commitment period applies to a regular switch between statutory health insurance providers. In addition, the switch must be initiated in a timely manner; typically, there is a two-full-calendar-month notice period ending at the end of the month.

If the supplemental premium increases, special options for switching plans may apply. If you have chosen a specific plan, additional lock-in periods may also apply.

Therefore, you should not wait until the next premium is due to consider switching.

Frequently Asked Questions About Health Insurance for the Self-Employed

<details class="wp-block-details is-layout-flow wp-block-details-is-layout-flow">
What will be the maximum GKV premium for self-employed individuals in 2026?

When applying the average supplementary contribution rate of 2.9% set for 2026, the maximum contribution to statutory health insurance for self-employed individuals without entitlement to sick pay is €982.31 per month. If they are entitled to statutory sick pay starting in the seventh week, the amount is €1,017.19.

Long-term care insurance is added separately in each case. The actual health insurance premium may also vary because each health insurance provider sets its own additional premium.

</details>

<details class="wp-block-details is-layout-flow wp-block-details-is-layout-flow">
What happens to my health insurance if the startup grant is approved?

When you begin working as a full-time self-employed person and your ALG I benefits end, the Federal Employment Agency will no longer pay your health and long-term care insurance premiums.

If you were previously covered by statutory health insurance, your membership will generally continue as voluntary statutory health insurance, unless you withdraw in accordance with the statutory requirements and provide proof of other health insurance coverage.

Therefore, be sure to notify your health insurance provider in a timely manner about when you will begin working as a self-employed person.

</details>

<details class="wp-block-details is-layout-flow wp-block-details-is-layout-flow">
Does the startup grant cover my health insurance?

The Federal Employment Agency does not cover your actual health and long-term care insurance premiums separately while you are receiving the startup grant.

The Start-up Grant includes an additional €300 per month for social security for six months, on top of the most recent unemployment benefits received. In a possible second funding phase, €300 per month is paid for social security for up to nine additional months.

As a self-employed person, you must pay your own health and long-term care insurance premiums and factor them into your financial planning accordingly.

</details>

<details class="wp-block-details is-layout-flow wp-block-details-is-layout-flow">
As a self-employed person with a low income, do I still have to pay a minimum contribution?

Yes. In 2026, self-employed individuals who are voluntarily enrolled in the statutory health insurance system will be subject to a minimum assessment base of €1,318.33 per month.

Applying the average additional contribution rate of 2.9% results in a minimum monthly health insurance premium of €222.80 without entitlement to sick pay, or €230.71 with entitlement to sick pay.

Long-term care insurance is an additional cost. Here, too, the actual amount may vary depending on the additional premium charged by your health insurance provider.

</details>

<details class="wp-block-details is-layout-flow wp-block-details-is-layout-flow">
Is private health insurance worth it for the self-employed?

There’s no one-size-fits-all answer to that. Factors such as your age, your health, the benefits you want, your family situation, and your long-term financial plans all play a role in your decision.

It is particularly important not to compare only the current premium. Switching back from private to public health insurance at a later date is possible only under certain legal conditions.

You should therefore view the decision to purchase private health insurance as a long-term commitment and, if necessary, discuss it individually with a qualified insurance advisor.

</details>

Conclusion: Plan for health insurance properly from the very beginning

There are a few special considerations regarding health insurance for the self-employed that you should take into account before you begin your full-time self-employment. Especially if you’re starting a business while receiving ALG I benefits and a startup grant, you should clarify early on how your insurance coverage will continue and what costs you’ll incur.

Whether voluntary statutory health insurance (GKV) or private health insurance (PKV) is the better option depends on your personal situation and your long-term plans. Above all, it’s important not to focus solely on the current monthly premium. The premium amount, sick pay, potential family coverage, and the long-term consequences of switching systems should also be factored into your decision.

When planning to start your business, you should therefore realistically estimate your health and long-term care insurance premiums from the very beginning. This will help you avoid a situation where a significant recurring expense only becomes apparent after you’ve launched your business.

Do you want to plan your self-employment carefully with a startup grant?

Health insurance is just one of many factors to consider when starting a business.
